  • Hospice Care

    Hospice services are designed to help patients, their caregivers, and their loved ones; during treatment, family or friends act as primary caregivers, assisted by an experienced team of aides. A hospice caregiving team includes the patient's physician, a hospice physician, clergy or spiritual counselors, nurses, aides, therapists, and social workers. For patients living in assisted living facilities, the hospice team coordinates with caregiving staff to provide senior home care services. Emphasis is on the physical, mental, and spiritual well-being of the patient; once the patient passes, hospice provides emotional and spiritual support for his or her loved ones.

  • Hospital Coordination

    We work tirelessly to keep you out of the hospital, but sometimes your condition requires hospital visits or stays. When the hospital level of care becomes necessary, our team collaborates with your physicians at the hospital on each decision regarding your care, every step of the way. We track your progress, when you are likely able to leave the hospital and work to ensure a smooth transition back to wherever you call home.

Our wound care services include keeping the wound clean, warm, and moist to provide the best healing capabilities.

    Our nurses will help you change the dressing and provide information on how long to keep each dressing on. We will help you clean the wound and apply a new dressing, all while making sure that it is healing correctly.

    When you leave the hospital, there can be uncertainty about your next step in recovery—when to take medications, how often you will need rehabilitation or what to expect in terms of regaining your ability to complete daily tasks.

    When you are ready to leave the hospital, we work with your physicians to determine the best level of care for your condition. We work together to set up all needed services including visiting nurses, physical therapy, primary care follow-up visits, home health services, and more.
